we usually say "высосаный из пальца" when something is very insubstantial or made up even, yet someone tries to make it look real and important

Yes OUT OF THE BLUE on its own means SUDDENLY, WITHOUT WARNING. But it can also refer to actions taken on the fly, without foundation, etc.
