THE WHAT (NOW)? is used to question a noun that you didn’t understand or aren’t familiar with: A: In order to get to the battery, you have to open the cruckshank. B: The what (now)?
Asking “The what?” simply means, what does that [noun] mean? Adding NOW at the end gives the statement a nuance of “Let me see if I understand this correctly… the what?”
