This refers to someone the speaker doesn't like, who obviously has a big round head/face and is a regular or recurring feature in their life.

"ole" is a form of "old" used to indicate a person (or thing) that is a fixture - you encounter them on a regular basis, or they turn up at regular intervals in your life like a teacher, neighbor, someone who works at a store or restaurant you frequent and it expresses some disdain. For instance, if Uly left Lingvo for a while and then came back, one could say "I got my first nasty comment in a long time from ole Uly this morning. I guess he's back! (ugh!)"
