Example
I have been thinking and have decided that we haven't done all what we had been planning."
I have been thinking and I have decided that we haven't done everything we planned/have been planning."

I pondered it over for a while and it dawned on me that we hadn't accomplished all we'd planned.

I just came to the conclusion that we didn't do all that was planned.
Translator's comment
As on option, you can use "we hadn't done". Also you can say "I've just come..",

I would recommend to google "conjunction reduction" if you want to find the answer to your 2nd question.
