Possible interpretations: if only I had studied harder, I would have passed the test. Or If only I studied harder, I would get better grades in school. Which one applies here?

As you see, I translated your interpretations. Russian doesn't have the types of conditionals English has, so если бы я учился усерднее can imply both the past and the present. It's only context where you see what time does the action refers to.
