The speaker's attitude is: "Why don't you ever smile? Is it because there's nobody in your life to make you smile... or perhaps there IS in fact somebody, but they don't know how to make you smile?" I used вдруг to render "perhaps" - is that not the right usage?

By the way, the speaker isn't saying this because she's smiling, he's wondering if she has anything in her life to bring her joy, like children, dogs, a husband, etc. If she were smiling when he said this, he would have said "Who IS MAKING you smile." But he uses the plain, general verb to refer to the existence or absence of someone to make her smile.

Думаю, это лучше всего передать через "кто-нибудь" вместо "кто"

Однако, не было бы это "is there anybody who makes you smile" по-английски?

It's hard to explain. Let me illustrate it in a snippet:

A conversation between two women coworkers:
Betty: You're so pretty - why are you still single, if you don't mind me asking.
Anne: Thanks. I don't know... I guess I'm not the marrying kind.
Betty: That seems kind of sad... nobody to share your joys with, coming home to an empty house every day, nobody to grow old with. Who encourages you when you want to give up? Who stands by you when the whole world is against you? Who makes you smile?... or is nobody up for the job? (=are you so picky that nobody is good enough -or- able to do those things for you?)

Uly, these translations in context.reverso may be correct in some context, but generally nobody would translate 'perhaps" and "maybe' by " 'вдруг'. The word вдруг suggests some guess and surprise.
